fix(hub-guide): correct hooks.json format and remove redundant manifest field

- hooks.json needs 'hooks' wrapper key at top level - Remove 'hooks' field from plugin.json (auto-discovered from hooks/hooks.json) - Simplify hook entries (no nested hooks array)
